Official Rules
• NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.
• PROMOTION OVERVIEW: The 2016 Laredo Hospitality’s The Dog Days of Summer Promotion (the “Promotion”) is sponsored by Laredo Hospitality Management, LLC (“Sponsor”). The Promotion will be available in forty-one (41) participating gaming cafes/restaurants that operate in Illinois under the ownership and management of Sponsor under the names Stella’s Place and Shelby’s (the “Gaming Cafes”). Each participating Gaming Café will have one hundred (100) Promotion passports (the “Passports”) available for distribution on a first come, first served/requested basis to those who meet the eligibility requirements set forth in Rule 3, below, for a total of 4,100 total Passports available to be distributed across all participating Gaming Cafes. Limit one Passport per person. You must have a Passport to participate in the Promotion. The Promotion consists of six (6) different weekly challenges conducted at each Gaming Cafe (each a “Challenge,” as detailed in Rule 5, below), six (6) weekly drawings conducted at each Gaming Café (each a “Weekly Drawings,” as detailed in Rule 6, below) and two grand prize drawings conducted from among all eligible entries that all Gaming Cafés have received for each Weekly Drawings (the “Grand Prize Drawings,” as detailed in Rule 6, below).
• ELIGIBILITY: The Promotion is open only to legal residents of the state of Illinois, Wisconsin and Indiana who are twenty-one (21) years of age or older at time of entry, a member of the Gaming Café’s playspinwin player rewards program*, and who are located in Illinois at the time of the participation in the Promotion (“Eligible Entrants”). Starting on Tuesday, July 12, 2016, the first one hundred (100) Eligible Entrants at each participating Gaming Café to request or receive a Passport shall be deemed “Participating Eligible Entrants.” Limit one (1) Passport per Eligible Entrant. No purchase of any kind, or gaming of any kind, is required for an Eligible Entrant to receive a Passport, but if an Eligible Entrant is not a seated guest, such Eligible Entrant must request a Passport from a guest service host or manager at the Gaming Cafe. You must be an Eligible Entrant to receive Passport and you must be a Participating Eligible Entrant to participate or receive/win a gift/prize in the Challenges, Weekly Drawings or Grand Prize Drawings. The following are ineligible to participate in the Promotion and/or to receive/win a Gift/Prize, even if they otherwise meet the definition of an Eligible Entrant: employees, officers, directors, agents, successors and assignees of Laredo Hospitality Management, LLC, its parent, subsidiaries and affiliated companies, its advertising agencies, the Gaming Cafes, and any entities or persons involved in any way with the creation, administration or fulfillment of the Promotion, and the immediate family members (defined as spouse, parent, child, sibling, or grandparent; whether biological adoptive, step or in-law) and those living in the same household (defined as those persons whether related or not who live in the same residence as the Eligible Entrant at any time during the Promotion Period) of such employees, officers, directors, agents, successors and assignees. Void elsewhere and wherever prohibited or restricted by law.
*To become a playspinwin player rewards member, you must meet the requirements for the program and complete an official registration form, which includes providing your complete name, street address (including city and state), date of birth, phone number and email address (optional). There is no fee to enter the Gaming Café and no fee to register for a playspinwin player rewards membership.
• PROMOTION DATES: The Promotion will begin in all participating Gaming Cafés on or about July 12, 2016 at 8:00 a.m. and will end in all participating Gaming Cafés on August 26, 2016 at 7:00 p.m., with intervening start and end dates for the Challenges (the “Challenge Entry Periods”) and intervening start and end dates for the Weekly Drawings (the “Weekly Drawings Entry Periods”), each as detailed in Tables 4.1 and 4.2, below. All times for this Promotion are Central Daylight Savings Time.

• CHALLENGE DETAILS: Each week of the Promotion there will be a new Challenge, as set forth in Table 5.1, below. Each Challenge will have its own page in the Passport (the “Passport Challenge Page”). Participating Eligible Entrants who complete a Challenge during the Challenge Entry Period for that Challenge are eligible to receive the stamp and employee signature (collectively referred to as a “Stamp”) from an authorized employee at the Gaming Café where the Challenge was completed and on the Passport Challenge Page associated with that Challenge. No purchase or gaming is required to receive a Stamp. When the Stamp or Stamps required for a Challenge are received, the Participating Eligible Entrant may claim his/her Challenge Gift at the Gaming Café from which the last Stamp for that Challenge was received, while supplies last, and by 6:45 p.m. on August 26, 2016. Limit one Challenge Gift per person per Challenge. Only 1 Stamp allowed per day, regardless of how many Challenges or steps in a Challenge are completed that day, and regardless if you visit different Gaming Cafes. Challenge Gifts not claimed by 8:00 p.m. on August 26, 2016 will be forfeited. At the time the Challenge Gift is claimed, the Participating Eligible Entrant must also enter the Weekly Drawings associated with the entry slip on which the Stamp appears (as detailed in Rule 6, below), or the Participating Eligible Entrant will forfeit his/her opportunity to enter that entry slip for that Weekly Drawings. If the Participating Eligible Entrant prefers not to enter the Weekly Drawings, he/she must forfeit the entry slip at the time of Challenge Gift claim.

Note that you may complete a Challenge at any time during the Promotion Period after the start date/time for that Challenge and receive the Challenge Gift associated with such Challenge as long as claimed by 8:00 p.m. on August 26, 2016. But if you complete the Challenge after the end date/time for the Weekly Drawings associated with that Challenge, you will not be eligible for that Weekly Drawings.
• WEEKLY DRAWINGS AND GRAND PRIZE DRAWING DETAILS: Each week of the Promotion there will be a Weekly Drawings conducted at each of the participating Gaming Cafes. To enter a Weekly Drawings, Participating Eligible Entrants who have received all Stamps required for any Challenge on the Passport Challenge Page for that Weekly Drawings Entry Period may complete the entry slip on that Passport Challenge Page with the Participating Eligible Entrant’s full name, phone number and email address, and deposit the completed entry slip into the Weekly Drawings entry box at the Gaming Café where the Stamp on that entry slip was received. No other form of entry into the Weekly Drawings or Grand Prize Drawings is allowed. The entry slip must not be deposited into the drawing entry bowl before the Participating Eligible Entrant claims the Challenge Gift associated with that Passport Challenge Page, or he/she will forfeit his/her claim to the Challenge Gift. All entry slips including the weekly drawing winners will be included in any remaining Weekly Drawings as well as in the Grand Prize Drawings (unless it is selected as the winner of the first Grand Prize Drawing, in which case it will not be re-deposited in the entries for the second Grand Prize Drawing). In order to be eligible for the Weekly Drawings and Grand Prize Drawings, the entry slip must contain the Stamp for the Challenge associated with that entry slip and be completed in its entirety. Entries become the property of Sponsor and will not be returned.
At approximately 7:00 p.m. on the Weekly Drawing Entry Period End Date, each participating Gaming Café will conduct a random drawing from among all eligible Weekly Drawing entries it has received up to 6:45 p.m. that day to select two (2) potential Weekly Drawing Prize Winners of the Prizes detailed in Table 6.1, below.

Odds of winning a Weekly Drawing Prize depend on the number of eligible entries received at that particular Gaming Café as of the end of the last Weekly Drawing Entry Period.
The total ARV of all Weekly Drawing Prizes per Gaming Café is $270 ($11,070 across all participating Gaming Cafes).
At approximately 7:00 p.m. on Friday, September 2, 2016, all eligible entries received from among all participating Gaming Cafes Weekly Drawings will be combined for two Grand Prize Drawings to take place at the Gaming Café located in Alsip, Illinois.

*Additional trip details: Trip includes round trip coach air transportation from major airport nearest winner’s home to Las Vegas, Nevada and double occupancy hotel accommodations at hotel determined by Sponsor. Both winner and guest must be 21 years of age or older. Meals, ground transportation, tips, gratuities, baggage fees, spending money and any other expenses not specified herein as being covered by Sponsor are the winner’s sole responsibility. Winner and guest must provide proper travel documentation prior to booking, and may not be on the no fly list. Winner will be required to present a valid credit card at time of booking and at check in to cover any expenses incurred that are not included in this prize description, as well as any damage to the hotel room. Exact travel dates and arrangements are subject to availability and blackout dates. Trip guest will be required to sign a liability/publicity release prior to ticketing. Trip must be completed by March 2, 2017 or it will be forfeited. If actual value of trip is less than stated ARV, no cash equivalent will be made.
**Additional Free Play for a Year details: Free Play for a Year will be awarded in the form of a $10 credit per day for one year (365 days) to be used at any of the participating Gaming Cafes. The winner will be able to redeem up to $10 each day of the year starting on the day that the prize is claimed (per the winner verification details set forth in Rule 7, below). If a day is not redeemed, the $10 credit for that day will roll over into the next day. A day’s credit cannot be used before that day. Examples: If the winner claims his prize on September 5, 2016, on September 6, 2016 he will have one $10 credit. If he does not use that credit, on September 6, 2016 he will have two $10 credits, or $20 in credits. If he does not use any credits until September 25, he will have twenty $10 credits, or $200 in credits. On September 25th, the winner cannot use any more than $200 credits.
The ARV for the Grand Prize Drawing depends on the prizes selected and ranges from $2,500 to $3,650.
Odds of winning a Grand Prize depend on the total number of eligible entries received for all Weekly Drawings throughout the Promotion Period and from all Gaming Cafes during the Promotion.
• WEEKLY DRAWING AND GRAND PRIZE DRAWING WINNER NOTIFICATION AND PRIZE CLAIM PROCEDURES: Potential winners do not need to be present at the time of the drawing. Potential winners from each Weekly Drawing and the Grand Prize drawings will be notified by email at the email address provided on the entry slip (the “Winner Notification”).
• Weekly Drawings Prize Claim Process: Potential Weekly Drawing winners must come to the Gaming Café at which their entry was selected within fourteen (14) days following the send date of their Winner Notification to verify eligibility and pick up their Weekly Drawing Prize. To claim a Weekly Drawing prize, the potential winner must show a valid identification card (either a driver’s license or state ID) with the potential winner’s picture, name, address and age. Potential Weekly Drawing Prize winners may (in Sponsor’s sole discretion) be required to complete, sign and submit a Prize Claim Form which includes a liability and publicity release, where legal, before they will be verified as the winner and awarded the prize.
• Grand Prize Drawing Prize Claim Process: Potential Grand Prize winners will be sent an Affidavit of Eligibility and Prize Claim Form (including a liability and publicity release, where legal, and a designation of which prize – the featured prize or the cash alternative – the potential winner is choosing) attached to their Winner Notification. Potential Grand Prize Winners must complete, sign and return to Sponsor the Affidavit of Eligibility and Prize Claim Form by mail, as directed in the Winner Notification email, within fourteen (14) days of the send date of the Winner Notification. Following Sponsor’s receipt of the completed Affidavit of Eligibility and Prize Claim Form, Sponsor will contact winner with details on how to redeem his/her prize. If the first Grand Prize winner selects the trip prize, he/she will be sent a certificate with details on how to schedule the trip. If the second Grand Prize winner selects the “Free Play for a Year” prize option, the claim date of the prize will be the date that the Sponsor sends the winner the redemption details (as evidenced by the postmark date if sent by mail or courier, or the send date if sent by email). If either Grand Prize Winner selects the cash alternative prize, it will be mailed to him/her within approximately four to six weeks following verification.
If Winner Notification or Prize is returned as undeliverable, if potential winner fails to claim the prize as required by these Official Rules, if a potential winner is determined to be ineligible or not in compliance with these Official Rules, if Affidavit of Eligibility and/or Prize Claim Form is not received within the time frame established herein or is not completed in its entirety and sign/notarized (if required), the potential winner will be disqualified and will forfeit his/her claim to the Prize. Sponsor reserves the right, but not the obligation to conduct subsequent drawings from the remaining eligible entries to award any unclaimed Prizes, but any Prize that remains unclaimed after at least one attempt to award it may not be awarded.
• CONDITIONS: All taxes on Gifts/Prizes are the sole responsibility of the winners, and winners of Prizes of $600 or more will be required to provide tax identification information and will be sent a 1099. Passports are not transferable and only the person to whom the Passport is issued can complete the Challenges and/or enter the Weekly Drawings or Grand Prize Drawings. Challenge Gifts and Prizes are not transferable. No substitutions of Challenge Gifts or Prizes except by Sponsor in the event that the featured Gift/Prize becomes unavailable. One Passport per Eligible Entrant for the entire Promotion. Sponsor is not responsible for lost or stolen Passports, Passport Challenge Pages or entry slips. Only original Passports, Passport Challenge Pages, entry slips and Stamps are allowed. No copies or reproductions allowed. Sponsor is not responsible for lost, late, misdirected, incomplete, damaged or ineligible entries.
By participating, each Participating Eligible Entrant agrees: (a) to abide by these Official Rules and decisions of Sponsor, which shall be final and binding in all respects relating to this Promotion; (b) to release, discharge and hold harmless Sponsor, the Gaming Cafes, and affiliated agencies or entities working on this Promotion, and each of their respective parents, affiliates, subsidiaries, and advertising and promotion agencies, and the respective officers, directors, shareholders, employees, agents and representatives of the forgoing (collectively, “Released Parties”) from any and all injuries, liability, losses and damages of any kind to persons, including death, or property resulting, in whole or in part, directly or indirectly, from entrant’s participation in the Promotion or any Promotion-related activity or the acceptance, possession, use or misuse of any Challenge Gift or Prize, including travel if Grand Prize #1 travel prize is selected; and (c) in the case of winners in the Weekly Sweepstakes or Grand Prize Drawings, the right to use such winner’s name, photograph, quote(s), and/or likeness for advertising, publicity and promotional purposes in any and all media, now or hereafter known, without limitation and without compensation other than the Prize awarded. By participating, entrant also agrees not to release any publicity or other materials on their own or through someone else regarding their participation in the Promotion without the prior consent of the Sponsor, which it may withhold in its sole discretion.
• MISCELLANEOUS: Unclaimed Passports will not be distributed. In the event of a discrepancy between these Official Rules and any creative/collateral materials relating to the Promotion, these Official Rules will prevail. Released Parties are not responsible for lost, late, incomplete, damaged, inaccurate, stolen, delayed, misdirected, undelivered, or damaged entries, or for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Promotion, including, without limitation, errors or difficulties which may occur in connection with the administration of the Promotion, the notification of winners, or in any Promotion-related materials. No more than the advertised number of Challenge Gift and Prizes will be awarded. Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by site users, tampering, hacking, or by any equipment or programming associated with or utilized in the Promotion. Released Parties are not responsible for injury or damage to any person’s computer related to or resulting from participating in this Promotion. Persons who act in an unsportsmanlike or disruptive manner or who are in violation of these Official Rules, as solely determined by Sponsor, will be disqualified and all associated entries will be void. Sponsor reserves the right at its sole discretion to suspend, modify or terminate the Promotion (or portion thereof) for any or no reason, including if Sponsor determines that any portion of the Promotion is compromised for any reason, and, if terminated, at Sponsor’s discretion, Sponsor may select the potential winner(s) for the drawing(s) at issue from among all non-suspect, eligible entries received for the applicable drawing(s) prior to action taken. CAUTION: ANY ATTEMPT TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE PROMOTION MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK ANY AND ALL REMEDIES AVAILABLE FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION.
• ARBITRATION: Except where prohibited by law, as a condition of participating in this Promotion, Participating Eligible Entrants agree that (a) any and all disputes and causes of action arising out of or connected with this Promotion, or any prize awarded, shall be resolved individually, without resort to any form of class action, and exclusively by final and binding arbitration under the rules of the American Arbitration Association and held at the AAA regional office nearest the Sponsor; (b) the Federal Arbitration Act shall govern the interpretation, enforcement and all proceedings at such arbitration; and (c) judgment upon such arbitration award may be entered in any court having jurisdiction. Under no circumstances will a Participating Eligible Entrant be permitted to obtain awards for, and Participating Eligible Entrant hereby waives all rights to claim, punitive, incidental or consequential damages, or any other damages, including attorneys’ fees, other than Participating Eligible Entrant’s actual out-of-pocket expenses (i.e., costs associated with participating in this Promotion), and Participating Eligible Entrant further waives all rights to have damages multiplied or increased.
• CHOICE OF LAW: All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrant and Sponsor in connection with the Promotion, shall be governed by, and construed in accordance with, the substantive laws of the State of Illinois, USA.
• REQUEST FOR WINNERS’ NAMES: For the names of the major prize winners (those with a prize value of $25 or more) (available after September 16, 2016) send a self-addressed, stamped, envelope by October 30, 2016 to: Passport Promotion Winners’ List, 2700 South River Road, Suite 106, Des Plaines, IL 60018.
